The following notice form is to be filled in and submitted whenever there is a repair or construction project in any house in our community.  This is an electronic form, there is no need to print it.  See below for details.

 

Fill in instructions:

1) Fill in name and house #

2) Check on one or more fields as needed.

3) Provide a description of the proposed works, to give us a clear idea of the proposed works. (Ex: painting exteriors;   Redo kitchen floors, etc.)

4) Fill in, cost of the works, duration, start date, and contractor’s name.

5) Fill in the date and

6) PRESS SUBMIT   (upper left button or bottom right)

This form would get to us automatically by email, if you need a copy, please let us know and we will email you back a copy of the submitted form.
